Still needs to be said though. Women arent always as shallow and superficial as us. It's not always about looks for them. He may think she's out of his league but she probably sees some other mysterious stuff in him, who knows.

Now the question becomes do I take my usual 4mg dose of xanax in the morning as per usual or at night before the date? The other thing, I open up way more easily when I've had a drink in me...

I was in a similar situation a little over a year ago. Met a girl for a first date at a sushi place. She's beautiful, a semi regular marathon runner, has her MD, and a few weeks later was accepted to Stanford Medical for a fellowship. I, on the other hand, can barely make it through a 5k, have a job that doesn't require a college degree. I'm a bit overweight, not the best looking guy in the world for sure. But, I have a good heart and I treat her right. We're still together, and I couldn't be happier.
What I'm trying to say is, its up to her to decide if you're in her league or not. Don't be so preoccupied with her looks. In the long run, looks don't matter nearly as much as personality and common interests. If that's all she's got going for her, take those borderline NSFW pictures, post them here so you'll always know where to find them, grab the crusty sock from the bottom of the hamper and let nature take its course. If there's more to her and you find some common interests and stuff to talk about then tell her you've had a great time and you'd like to do it again. And definitely be yourself. If the two of you spend enough time together she's going to learn the real you anyway. You can't hide who you really are forever. And that's who you want her to fall for, not some trumped up image designed to impress her.

You have a few negatives coming into this that you should be wary about:
1) Your attitude. As others have said, the fact she agreed and is excited about the date is all you need to know. Banish the "league" thinking and treat her like you would any other girl. I know quite a few very attractive women and a lot of them have the most messed up relationships ever. Only the supremely cocky guys get the guts to ask these girls out and the relationships predictably suck. You will be different than these guys because you are different. Those guys don't make threads about pre-date jitters. Different is good because whatever she's been looking for previously hasn't worked out for her.
2) Your recent relationship. Be careful talking about ex's. As a matter of fact, specifically do not mention anything about your ex except to acknowledge you are indeed single. Nothing turns off people faster than hearing about ex's. You may find this comes out in strange ways, for example "Oh you know my ex didn't like shrimp either." You may think this is innocent commentary, but for the other party they will just hear "my ex my ex my ex" all night long. But if she wants to talk about her ex's then let her talk. A girl's favorite topic is herself. The more she gets to talk the more she feels like she's bonding with you even though you aren't really doing anything. You also get to learn more about her. It is a win/win.
Finally, some general dating pointers I have racked up:
1) Have some backup ideas of what to do or where to go. Then if something doesn't work out or you two decide to extend the date, you're not stuck desperately trying to figure out where to go. Don't plan in minutia but at least walk through some options mentally. And then if it does go to the backup plan, you seem like a guy who is in charge and connected with the town/scene/life.
2) Self-deprecating humor is good if used sparingly. It can be a turnoff if it goes too far though. Women like confident men who are well planted in their situation.
3) Watch your drinking. You said you loosen up after some drinks. Some mild to moderate alcohol could help. But take it from a teetotaler who observes a lot of drinking (comes with the profession), a lot of people THINK they loosen up after some drinks but they're really just making idiots out of themselves. So don't go overboard.
