This is almost a no-brainer.
A graphics card based on the Radeon 4770 HD chip.
Fantastic bang for the buck, and you can SLI two of them together!

Depends on what the manufacturer build onto the card.
Example:
The Asus EAH4770 has an integrated HDMI out.
The Sapphire 100277L does not have HDMI out, yet both are based off the Radeon HD 4770 chip set.
Depends on your use of the cable. HDMI carries not only video, but audio as well. If you want to stream something, such as a movie, to your TV with only one cable then HDMI is the way to go.

I recently bought an EVGA 9800GT. I wanted to keep it under $100. After a month or so of playing with it I almost wish I went with a more expensive card (or even a Radeon). I was coming from a 8600GTS. The gains were pretty noticable, but in all honesty I was hoping for some better performance (e.g. Assassin's Creed still lags quite a bit). I have an E6600 and a 680i.

4350 would be great for HTPC use, but not for gaming. Not fast enough. What about 9800GTX+? You can snag one of those for under a hundred. Heck if you catch the right deal, you might be able to find a GTX260 for just over a hundred.
