How to find physician who could do physical checkup required for SCCA/NASA license?
A friend of mine (in Illinois) is interested to apply for his NASA driver's license. Together with his application, he is required by NASA to submit a form filled out by a physician stating he is fit to drive. I have not seen the form myself but I assume the test involves physical, mental and tolerance to conditions the driver endures while driving (such as heat).
Can anyone give me some tips I can pass on to my friend as to how he can find a physician that could perform the necessary physical test? Also, how much should he expect to pay?
Thanks in advance

From what I've heard its just a standard physical that your regular physician should be able to fill out.
They (NASA) have a forum too:
