If either of them fail it can cause the timing belt to fail. IMO, replacing these items is almost as important as the belt itself. Plus, if either one goes out, it's going to cost almost as much labor to fix as it does to do the timing belt. If you do it all at the same time, you're getting the tensioner and water pump nearly labor free.

I did my j35 Pilot this summer and it was worth it to do it all at once. I changed everything even though it wasn't totally worn out.

man i was just there like a week ago, i was thinking if i should change the pump too and i talked to the honda tech at my local dealer and they said they would do it for no labor cost because i was replacing the belts and the tensioner already. so it is a great peace of mind to have it all done, specially only paying the one time labor fee. there is a thread on here where you can buy the whole package for under 300 bucks from a acura dealer search for it. luckily i got mine from a member on here for 240. belts, head gaskets, 4 qrts. acura tranny fluid, oil filter, cabin filter, waterpump, tensioner, and some other seals. it was a sick deal all were packaged in original honda packages. but look for that thread and do it all at once man, it saves you money in the long run.

You always do both of them while you do the timing belt. if your pump were to fail you will have to pay all that labor for them to get to the pump and replace it. And why not just do it, its one less thing to worry about!

I would do the tensioner. I did not do it with my timing belt and 25k later I ended up replacing it because it started failing in cold weather. (Dealer was kind enough to waive a majority of the labor since they are the ones who had done the timing belt/water pump, but I wouldn't count on that.)
