Hey everyone. We are buying out our 3 year lease on our 2020 MDX in about two weeks.
What is everyone doing in the way of extended warranties, if anything? We have the option of an Acura certified warranty for an additional two years and up to 100,000 miles at a cost of around $2000 (this option includes 1.9% financing for up to 60 months). Or we could forego certification, and then we have one more year before the factory warranty expires to decide whether to buy an Acura care extended warranty (dont know prices yet).

They are the same warranty (different checkbox on the same contract).
See prices at online Acuracare sellers. They take very little comission and are usually way cheaper than what you can get from a local dealer.
So go with whatever is the better price. (Usually online)

What is the model you have (base, tech, Aspec, Adv, hybrid)? Do you live in extreme climates with temps above 110 F and/or single digits in winter? Are you planning to keep the MDX after the CPO/Acuracare expires?
I did an extra 5 years and up to 120,000 miles for my 19 MDX Adv Sport Hybrid a few months before the 4/50k basic expired. I'm planning to keep my MDX until +/- 150,000 miles. I just did my local Acura dealership for around $2500 (There was $200 extra charge because of my mileage being over 40k. I live in the southwest with summer time temps that can kill the Acurawatch camera over time. Plus, not a lot of Sport Hybrids over 100,000 miles and don't know the long-term issues with the EV components ($15,000 for 7DCT, $7500 for TMU, unique 3.0L V-6, DC inverter, battery pack, firmware/software, etc...).
The 3.5L 2020 MDX seems to be extremely solid vehicle. The price of Acuracare or CPO is the same for the base, tech, ASpec, Adv, or Sport Hybrid. The CPO process is an 182 point inspection and replacement of some wearable items like brake pads or tires if they are under a certain %. It was all the extra tech in the Advance Sport Hybrid model made me decide on Acuracare. I would have skipped it with a 3.5L MDX.

Thank you. Its a 2020 MDX tech. Ive already done the certification inspection and according to the report, they are not replacing anything, which is strange, because the car has 37,000
miles.

Doesn't sound like it is worth the certification process if there are only checks and nothing is being fixed? I figured at least the original tires might be close to +70% wear with your MDX with +37,000 miles. My OEM Conti LX Sport tires were replaced at 41,000 miles and they were in the 6/32nd-5/32nd range. You might save some money and get a longer warranty going with Acuracare in 4-5 year of coverage for up to 100,000-120,000 mile depending on plan. I "think" the most you can get with CPO is the 4/50k basic extended to 6/100,000 miles and the standard powertrain of 6/70k is extended to 7/100,000 miles. The CPO/Acuracare warranty extensions will not start until your 4/50k basic or 6/70k powertrain expires depending on the issue.
Example: My 18 RLX Sport Hybrid was purchased as a CPO in 2019 with 7200 miles. I only have under 30,000 miles today; but, the CPO 6/100,000 miles warranty will expire sometime in 2024 and 2025 for the CPO 7/100,000 powertrain extended warranty. I would be still covered until 2027 if I went for 5yr/120,000 mile Acuracare with the 4/50k basic still active. I'm covered until 2028 (or 120,000 miles) with my 19 MDX Adv Sport Hybrid with the 5/120k Acuracare I just purchased last Feb/23.

I have a 2019 MDX Aspec with 84,000 miles.It has been problem free except I needed a new camera for all of the safety features which was about $1800 CDN. Maybe the warranty is worth it for you given your low mileage. I did not get it.

I purchased the acura care warranty from curry acura for my mdx, I paid less than that, but my warranty only goes up to 80,000 miles.
I don't put a lot of miles on my mdx and won't reach 80,000 miles before the time limit expires on the warranty, which is why I went for the warranty with fewer miles.
