I know with the aftermarket leather kits sold at www.leatherseats.com (think thats the address) they have inserts for our doors. the are pre-folded to get the nice look ruffled stock look and just tuck under the sides of the plastic door skin and glue down to the foam behind the cloth.

Oh do you really only have 3,300 miles on your car or is it 33,000? Either way thats pretty amazing to compared to my 191,000

I've had mine off several times, but I didn't pay too much attention to how the leather portions were attatched.

They come off pretty easy. There are several screws and push in clips. Just look around the panel, and they become obvious (I think 1 or 2 of the screws are "hidden" under plastic plugs). Once the panel is off, you'll need to unplug the door lock switch, window switches, etc. in order to remove the panel completely.
