Tranny shifting smoothy 1st - 3rd. Wont shift into 4th or 5th

Transmisson shifts smoothly 1st gear through 3rd gear but wont shift into 4th or 5th gear. Wondering if anyone know whats going on with my tranny. Its a 2001 cl-s with 150k. Thanks for any sugestions.
