They are given out by police officers to close friends and family. This article explains it well:

As a word of warning, these cards are not get out of trouble free cards. some officers might cut you some slack, others will not. and if an officer questions you about your card and you don't give them the right answer, be prepared to pay out the ass. my uncle (who is a police officer and i have a PBA card from him) told me of a story of a fellow police officer who pulled over some guy who ran a stop sign. He was going to just give him a warning for the violation. But the guy had handed over his license, his insurance and a PBA card. The officer recognized the badge number (it belonged to someone he knew and he's asian) and the offender was white. He asked the offender two simple questions. First was, how did you get this card? The guy responded, he's my uncle. (Could be a reasonable answer) Then the officer asked, how Steve's (not the first name of the officer on the card) been. The guy answered pretty good. Immediately, the officer knew this guy had no connection to the officer listed on the card. Instead of giving the guy a warning as he intended to, he came back with 4 for him. One for running the stop sign, one for speeding, one for passenger riding without a seat belt on and one for the tinted windows.
I personally don't think getting the card is worth is, especially since you won't actually know the officer listed on the card.
