Looks like it's the whole door panel that needs to be replaced.

Look up places that repair dashboards...that can be fixed in the car. They will fill it with plastic and match the grain.

You might be able to pick one up from an auto recycler. Do a search for "Rear Door Trim Panel" on Car-Part.com--Used Auto Parts Market
