Problem for the Pros
A little history: 2005 Acura TL with mods; motor oil changed regularly with the good stuff...all fluids well maintained. Transmission blew around 63k, and had it replaced via good will - new internals, refurbished casing is what I was told. Now, here's the problem that I'm having; I'm not sure if it's related to having PCD's and the CEL that results from it from time to time 
Okay, here's the best I can describe it: you know how if you drive a manual car at really low speeds (without pushing in the clutch), the car tends to sort of shake because of the rpms are dropping so low? I have something of the same problem; except I'm an automatic. The problem is that my car seems to want to stall out when and only when I come to a stop; a QUICKER stop more seemingly produces a more harsh vibration/stall feeling than when I coast to a slow stop (no stall feeling at all). The rpms drop to about 300-400 rpms and jumps back up. I did the 105k roughly 2,000 miles ago - plugs, water pump, tensioner, etc. Is this something transmission related? I've yet to drain and fill my transmission oil in the last 13,000 miles, roughly

Sounds like the trans. is not down-shifting as it should causing the engine to lug down & stall (die or almost) Try it in manual mode & down shift yourself & see if it tries to stall.
Make sure trans. is full with correct fluid.
Have Auto-zone check for a problem code, TPS possibly.

The fix to this is VERY simple!!!!
Unplug the battery for 30 minutes, make sure you have the radio codes, presets and other stuff written down.
Then put the car in stall mode, turn off ALL accessories such as A/C, heater, lights, radio, etc.
Turn the car off, then turn it back on and let it idle for 15 minutes. Do not turn on any accessories or move the car. The car needs to have the idle relearn procedure done and this is causing the issue.

You are not off my friend! The voltage in the battery drops so low during startup that the ECU forgets the idle procedure and then does this exact stalling thing! The lack of the idle procedure in the ECU is what causes this issue for 90% of cars with this issue. In the other 10%, the problem is usually related to something else such as a clogged PCV valve...

If those don't fix it, start looking at the torque convertor clutch. I've had them on other cars not want to unlock and most autos don't downshift to first gear until speed is under 5mph so it's just like pushing the clutch in at the last minute on a manual. I just had my friend's car do it to me in the middle of an intersection, killed the engine and I nearly got rear ended.
If those don't fix it, start looking at the torque convertor clutch. I've had them on other cars not want to unlock and most autos don't downshift to first gear until speed is under 5mph so it's just like pushing the clutch in at the last minute on a manual. Ya, sounds like a vacuum leak...like he said if the car does not "relearn" the idle procedure its probably your PCV valve...
Also just below your post was this:
discovered that failure code of P0404 (EGR Valve malfunction) had been recorded.
I removed the EGR valve and discovered that small pieces of carbon had logded the valve open, causing a lean idle condition. We cleaned out the carbon with compressed air and carburetor cleaner and ensured that the open-close action was smooth. We reinstalled the EGR valve and cleared the error code and she once again idled smoothly.

I agree. A sticky TCC solonoid could cause your issue as well.
