Please Help with Engine codes! Lean o2 sensors???

ok if anyone can help me out Id really appreciate any input from you gurus out there...about a week ago I started having a hesitation when I would accelerate normally and it felt like the car was losing power for half a sec every few seconds. I didnt have a check engine light so I thought it might be the egr valve and replaced it with a new one. That did not fix the problem and the next day the hesitation got worse and as i was driving the check engine light and vsa light came on and the car felt like it lost about 90% power. Today the car has power back but the check engine light is still on so i got the codes checked and it showed the following:
P2197- o2 b2 s1 signal stuck lean

p2138- app sensor a-b correlation

p2195- o2 b1 s1 signal stuck lean

Again, any help or advice is appreciated. The only mods iv done to the car is an AEM cold air intake and the mid muffler delete. The car is an 04 base 6mt with 120,000 miles. Car ran great before this happened although i did get a check engine light a couple of times before after installing the CAI but it would turn off the next day or so. When i checked the code those couple of times it said it was lean....

OK I got ahead of myself, after doin some searching I found that other ppl have had this problem with the code p2138 and replaced the app sensor and the problem went away but I did not see anyone who had all three codes...could the other two be a result of the p2138?

Was the valve adjustment and timing belt performed at 105K? It could be a bad 02 sensor and as well the APP sensor. It sounds like that the app sensor was going out first, and then after the EGR was replaced the 02 sensor went out since the gasses started recirculating.

does gen3 have the same gen2 issue
with plugged egr ports inside theintake manifold-about every 75kmiles need cleaning
Not the valve itself- the ports thru which the exhaust gas must flow get carbon plugged

Need to remove intake manifold from the engine- clean out ports with carb cleaner and wire hanger- no kidding
Clean the Idle air control valve under TB while its in your hands

codes are clues not diagnosis- you get many false codes and one that points to a problem
