Exclamation Grinding Sound!?!

I have an '04 TL with 75,000 miles, AT.

I sometimes get this grinding sound when I drive. I can't figure out what causes it or what it may be related to but it sounds most like it resonates from the wheels so I have it narrowed down to something to do with the brakes (which haven't been changed, ever)

I know that the sound never occurs when I'm going under 15-20 mph, but usually occurs when I'm going faster than that. My brakes do squeak when I press on them, but I know that's just because they need replacing.

The rotors aren't scratched or warped at all, from what I can feel with my bare hands and from what I can feel when I press the brakes (the car doesn't pull).

So what the hell is that sound?

It's driving me nuts and I'm worried it's something that's seriously damaging my TL.

Could it be the bearings in one of the wheels is bad? Are the bearings expensive?
