Seriously. 3hours and 42 minutes = 3.7 hours. 59 mile / 3.7 hours is 15.9 mph average. Considering it probably samples rather than a straight averaging the result of 15 (which is rounded) is perfectly in the range of what can be figured out in one's head.
That said it is odd at first though. I typically think all my miles are highway at at least 65MPH but my averages are well below that on the MID. All that slower time really eats at the average.

Dude...look how far you went in 3hrs & 42min. You must have been parked for a long time. You should just turn your car off when you come to a standstill if you are moving that slow (similar to the idea behind the Toyota Prius).
I was having similar problems. The stop and go, as people have posted above really does kill your mileage. Traffic in my neck of the woods (Minneapolis) can be bad if you hit it at the right time. When I averaged 25mph in my '07 type S, I was averaging about 17mpg. When I stepped my MPH up to about 45, I went up to 23mpg. Just took a road trip averaging 75mph and got 31mpg...and again in my 07 Type S which is listed to get from 19-28mpg.
It really depends on how you drive.

I didn't step the gas really hard, just very gentle. I thought the mpg suppose to be higher than 18 sth...I used to see that number when I only drive local.
I only refill the gas in costco with 91.
I think honda didnt do well on the MPG.....gas is more expensive right now...just want to get a CRZ sth....
And I didnt wash my intake filter for around half year already....anyone think I need to wash it asap? its around 4000 miles after the last wash.

you don't need to clean your AEM filter. they can go 15k before you need to do anything with them.
