Clunk Issue

When I put it into gear and start to drive off, there is a clunk noise once I let off the clutch. I can hear it twice and feel it in the pedals. As I change gears, seems less and cannot hear it at all. I have NO ISSUES when making turns, going over bumps, pulls, or low speed vibration. I do have vibration on the steering wheel if I am going about 65+; so the issue is in low speeds. Seems to be coming from the front driver side,

Searching around and trying to get a lead in the right direction; I came across these forums:

clicking/clunking when taking off and shifting into second - Honda-Tech

The infamous clunking noise... - Club RSX Message Board

Clunk going over bumps - Honda Accord Forum : V6 Performance Accord Forums

Suspension Mods:

A-Spec suspension
Fastline Compliance Bushing (note - I have never had an issue with these besides the squeak - fix)
XLR8 62a Front and Side Motor Mounts/OEM rear mount (All new)

Edit - I have inspected the ball joints, upper control arm, the shocks, endlinks, tie rods, front sway bar, transmission mounts, caliper......

Any advice and suggestions will be greatly appreciated.

Uh I guess it didn't post completely from my phone.

Anyway, the center mount bushings was missing a bolt and the other bolts were loose. I re-tighten all the suspension bolts in which I found that the upper control arm bolt was loose.

Overall check your bolts. I have been driving it for a couple of days and no more clunk noise.
