#5 spark plug failed causing damage to the cylinder head
Basically did the same thing although mine were first replaced at 120k, but same mileage for second set. They weren't loose but kind of surprised how little effort it took to break them loose.
Tips for time-sert
1. Ebay sells a Chinese endoscope web cam with leds that will fit down the spark plug hole. Great for finding chips that fall in and seeing the new spark plug seat is cut well.
2. Grease the tap
3. A small tube connected to a vacuum that you can put down the spark plug hole to vacuum up chips and a chop stick with grease on the tip to reach chips in the corners
4. Measure the distance from the spark plug hole to the top of the spark plug tube and mark that distance on the time sert tap so you don't tap too far down.
5. You can disconnect the fuel pump fuse so the engine won't start, then crank the engine with no spark plug in the repaired cylinder so chips can be blown out.

I was able to rethread the hole and sucked out what I assume all of the shavings from doing the job. Wondering if an oil change is necessary? Any thoughts?
