Brake hold releases with thud noise

When the car comes out of brake hold, I heard a loud nose from one of the wheels as the vehicle starts to move. What could be causing this?

Could it be the parking pawl? I've never heard a clunk from mine, but there does seem to be a lot of play in the system. If it were under load, or out of alignment with the gear, it might cause a noise when it retracts.

The car is in Brake Hold while still in D mode, like at a traffic light, and not in P mode. Would the parking pawl engage then?

Oh I completely misunderstood the question. I thought you were talking about the e-brake. I've used the brake hold and never heard a clunk that I remember, but I don't know how the brake hold works - I don't know if it uses the parking pawl. It doesn't behave the same as putting it in park does though. Seems to be more electronic.

brake hold applies the brakes, it does nothing to the transmission but shift to 1st gear

I have an issue with the car moving forward when the engine restarts. Maybe it needs adjustment?
