In my experience it is COP. But one not all. But OBD will not throw code for single one than for a few and usually one in middle is bad.

I would go and clean the EGR and the PCV and see if that helps at all. If you already checked all your plug gaps and rich/lean on the spark plugs, and did the coil isolation test, then EGR and PCV are easy checks/cleanings that could have decent returns on performance. Might want to even give that top end a good cleaning while you're in there. Good Luck!

Check for pinholes in the spark plug wires

I once had the problem at 3200RPM, I found a small hole in one of the plug wires that was arcing. It was very small and I had a small carbon trace on the plug wire that was very hard to see. I never did figure out why it needed 3200 rpm to act up. I changed it out and cleaned up the EGR channels in the intake manifold at the same time( as I was looking at everything I could think of) and the problem went away.

are you getting a CEL associated with the misfire? is it consistently the same cylinder, or random? there's a different course of action for each...
