i voted almost always,
I don't buckle up if i'm driving like <500ft. (ie. moving my car)
and sometimes when I'm a passenger in the backseat I forget to. but it's generally second nature, and I don't even think about it anymore.

Always, though I still havn't put them in my truck, it's on jack stands right now anyway.
I'm always mystified by my grandpa though, I've never seen him with one on, and he drives about 800 miles a week between here and his house in Mexico. I don't even feel safe on the 2 lane highways in Mexico with a seatbelt on. Usually they don't even have lines painted on the roads, and I doubt a ambulance can get to most of the places he drives in under 40 minutes.
