Abnormal engine knock/tick above 3k rpms

2012 Acura TL tech 3.5L 230k miles
I’ve developed a very strange tick or knock(not sure how to describe it.
it comes in around 3k rpms and is very audible at 3500 rpms. Still there at 4K rpms but doesn’t seem as bad as 3500 is.
it’ll do it in neutral or if the car is driving with a load on it.
This is not injector tick

sounds upper engine. the dealer adjusted valves but that didn’t fix it And now they advise to replace the engine. I’ve had the issue for about 2k miles and it doesn’t affect performance or anything other than a cosmetic noise.
I’m going to get a second opinion but wanted to see if anyone on here had a similar issue

check the oil for any metal flakes, same with oil filter. Check your spark plugs and make sure they are fully tightened down as the middle cylinders can loosen plugs over time and eject! IT could also be a belt noise but that'll require taking off the accessory belt first or checking the timing belt tensioner. With 230k miles, when is the last time you had your timing belt/water pump service done?

will do. I checked the dip stick and no metal flakes. But will go ahead and change the oil too.
I’ll check the other things you mentioned

timing belt done at 210k with a gated belt and asin kit with all components.

From the research I’ve done it seems like a very uncommon thing and haven’t found anyone with a similar issue.

It sounds a lot worse on video for sure though

Does it tick on idle? Open the hood and listen.
Mine sounded similar, ended up being a bad rear camshaft. Acura ended up replacing that under TSB/Goodwill, because they replaced the front camshaft during the oil consumption piston replacement.
