My iphone 12 pro max now charges after the latest iOS update. I still have to have the front of the iphone pointing to the rear of the car with the end of the iphone closest to the rear of the MDX kicked out to the passenger side, meaning the phone sits diagonally in the pad.

I checked today and my phone is also charging now. I do have to place it diagonally for it to stay charging. I have iPhone 12 Pro with an Apple silicon case.
I have difficulties too - On and Off charging - with my Google Pixel 4XL phone. Outterbox cover as well. It is weird that sometimes it works and others not. It is a slowwwww charger too. I Think (I will have to particular note), that If I'm using WAZE, it works more erratically. FWIW, I do use the car's Navi system which is very Good. Significantly better than the one in my former 2019 MDX. But Unlike WAZE, it does not detect speed traps, traffic Cameras, and slow to alter routes due to traffic accidents, etc.
In fact, last Saturday, travelling back home in Westchester County NY, from Sothern Jersey, there was 40 minute delays at the GW Bridge !!. WAZE took me a different way that I never knew existed and saved me from that Dread.

Do you have to sit the phone diagonally in the charging mat?

Hey - thanks to your post, I decided to try my 13 Pro Max on the charging pad again after not working previously...and it works! Much appreciated.
I don't have to orient the phone in any special way, though (like diagonal or "upside down," in your example). Mine sits normal...right side up and centered on the pad. Working like a charm.
