Lean Bank 2 for 11 Months!!

So I am officially sick and out of ideas for this damn code..

Here's what happens:
Atleast once a month the MIL comes on for lean bank 2 and goes away the next morning (3-6 starts later)

I have done (replaced) the following:
-bank 1 and 2 o2 sensors upstream (this did increase my mpg)
-all motor mounts
-pcv valve
-seafoamed twice
-all spark plugs
-only use premium gas
-smoke tested the far. And replaced any hoses that leaked. (only the boot leaked)
(all parts oem)

After doing all this the engine is very smooth but has a tiny vibration and once MIL is on it gets a bit worse.

I have a Bluetooth obd2 with the torque app and here are the fuel trim results with the lean code on

Bank 1 is 11%

Long term and short term fuel trims

No idea what's causing this light to come on and off for the past 11 Months.

I have an 06 TL Base, my bank 2 is always leaner than bank 1 (I use Torque also)....but I've never set a lean code.
Could be a fuel pressure thing on that bank, or possibly an injector issue (maybe variable cam too).
Do you log your Torque data? If so, maybe you can plot Bank 2 Trim (ST or LT) vs RPM and Load and see if this is happening somewhere or everywhere?

Also, what code do you set P0174?

Thanks for the Reply, P0174 is correct.
So i have the images of the Charts for RPM vs LTFT2 and RPM vs. STFT2
i also took a video of some sounds under the hood that are happening that i think are coming maybe from Valve's or lifters but im hoping someone knows what it is. I also dont see a pattern in the graphs.

RPM vs. STFT2

RPM vs STFT2 (try 2)

RPM Vs. LTFT2

Hmm, seems OK, bit of a wide scatter at 1500rpm. Video sounds OK to me but hard to tell.
Do you know if your local gas stations use ethanol blends in the fuel?

How often do you set the code? Do you have torque data when the engine sets the code and runs rough? What does the AFR measured say on torque?
