Check engine and VSA light keep coming on periodically

Hi guys. Ive had this happen on 3 different occasions, where my check engine and VSA light come on simultaneously and my car goes into limp home mode(cant rev above 3000 RPM's).

1st trip to dealer after check engine light and vsa came on and car went into limp home mode.--they did a "software flash" which fixed the problem, temporarily.

2nd trip-they "flashed" a differeent part of the ECU which again fixed the problem. Temporarily.

I am now going on my 3rd trip to the dealer. Codes pulled so far.
P2138
P2197
P2195
P2198

I believe my dealer has always done a good job overall but I am frustrated that this problem keeps coming up. My car is currently in limp home mode and I will be driving it in today for them to look at. Every time I have taken the car in, the check engine light is not ON at the exact time I get to the dealer(or the exact time they go to look at the car), so they said it is hard to diagnose. If thats the case I am not sure what the point is of the car holding a "history" of the codes.

Any suggestions from anyone who has had similar issues ? I have an 04 TL.
On a side note, the check engine light never came on once before I had my 105k service done, if that has anything to do with anything.

Thanks in advance for any replies.

serice bulletin 04-016 should take care of the p2195/97, if done correctly. this means performing the ckp relearn procedure too.

DTC P2138: APP Sensor A/B (Throttle Position Sensor D/E) Incorrect Voltage Correlation
if updateing the pgm-fi software and performing the ckp learn procedure doesnt fix this problem, replace the app sensor. yes the idle/ckp relearn must be performed after replacement of the app sensor.
my theory for your "limp" mode car, is a bad app sensor. think of it as your throttle position sensor. on my old civics i had a couple go bad or develop "soft spots" where no contact was made by the bar on certain areas. when i would pass by them my car would die down then shoot up then die back down.

i cant find anything for p2198.

SilverJ.... I'm having the *exact* same problem. I also have an '04 6 spd. My honda shop is having trouble w/ the diagnosis. The APP sensors were also brought up, but when they went thru the tests for the sensors, they checked out just fine....
Its intermittent with me too...hard to reproduce. Its happened 3 times in the past 2 weeks for me.
My dealer didn't say there were any ecm flashes even available....
Please let me know how you're addressing this issue.
send me a PM or email me at dob_01@hotmail.com if you can.
Thanks so much!
