1) Too much (About $4k for a rebuilt unit)
2) Don't do it. Get a used AV6 transmission. Much cheaper, easy install and will actually last, as opposed to a rebuilt TLS trans which will fail again, due to a faulty design. Gear ratios are just about the same. Look for an '06 or '07 from an V6 Accord. Search the threads here.

I paid $4K for mine. Don't do it. With that said, I had mine rebuilt at 96K (2011) and I'm currently at 190K going strong.
P0730 isn't typically associated with imminent transmission death like P0740 but could be a sign that you're approaching it. Did you have your transmission flushed? If so, you could have damaged it.
I would check your shift/lockup solenoids and pressure sensors and possibly replace the external transmission filter. Do another fluid change (drain and fill) and check the condition of the fluid.
