5th gear shudder

5th gear shudder
If I keep it in 4 it drives perfectly, soon as I slide it up to 5th or start out with it already in 5th it shudders so bad it feels like the car is coming apart. Any advice is greatly appreciated because needless to say she is not highway compatible.
