I DIY at like 50 to 75 K.
Flush with hardware installed to hook up hoses is overkill
and a way for service stations to markup a simple DIY.
The only case I can make for backflush is ... you waited to
long to replace and it got moldy oldie.

I did a preemptive flush at about 50K...but didn't flush out the engine too...just the radiator. Was SUPER easy. Just unscrew the plug at the bottom of the radiator and flush it out with a hose...then refill.
