If foam isn't working it might not be a cleaning issue?

I had a set of tires put on my BMW 325ic at Firestone about five years ago in Westwood, Ca. I was going to go to Long Beach on the freeway right after but did not. The next morning I went a few miles on the street from home and the front left tire fell off!

Luckily I was moving slowly so all four lugs did not release, only three so the tire leaned on the caliper and hub (ruining the bearing)

They replaced the damaged parts. I was on my way out of town the next day so did not pursue anything but they did give me a free set of tires. (I made sure the wheels were torqued properly)

This is the rim leaning on the caliper. I took this picture while I waited for their tow truck.

I am replacing a set of Yokohama YK 520s on Monday with 30k miles on them, they are at the wear indicator and are 60k tires. I am getting 50% of the 130 per tire I paid back but my feeling is I should have at least a year left on worn out tires.
