Assuming this is an exhaust leak?

I just want to make sure this assumption is correct. Last time I drove my '98 3.2 TL, halfway through the trip, the engine noise got amplified a LOT, especially when accelerating, but not so much when coasting. Is that definitely an exhaust leak? It got like this last time an exhaust pipe rotted out, but I just had a major repair in the block, so I want to rule that out. I really think it's the exhaust, but I've only listened to the noise and not done any special testing on it. Anything else it could be?

Only way to know for sure is find out where the noise is coming from.

The mechanic said the exhaust broke "flush with the flange". I am not sure what that is, but the sound seemed to be coming from just behind the engine, under the cab.

Sounds like it may be where your Y-pipe exhaust joins with the donut gasket and the spring system. It's very loud when that finally goes. It will be quite while coasting, but when you hit the gas, it's obnoxious. Not as bad as losing the muffler, but pretty close.
