These came off my car. 150k on them so I recommend a sand/polish/reseal. I can do the service for an added price (and its not a shitty meguiars headlight buff drill kit job either).
Driverside - no broken tabs, however the plastic where the high beam is, as well as the corner is cracked. Can be seen in photos.
Passenger side - tab nearest to hood is broken, but can be plastic welded back on. Am planning in doing that before selling.
No bulb covers, ballasts, igniters, or bulbs included.
