The way I see it is that single girls use it cuz they aren't getting the real thing.
Girls that use it and have a guy aren't fully satisfied with what he has to offer.
The only way I could see it being useful in a relationship is if the girl is down with DP.

As for who is in charge, considering it would be your two holes being penetrated, my guess is you would want to be in control. Maybe not the one actually controling the guy(s)/toy(s), but at least controling the controller.
