2016 MDX with sending package can't see car in front !
Used to work.
CAr can't identify that there is a vehicle directly in front of it. When I'm using the dynamic cruise control. It literally almost crashed right into the car in front of me. I've already cleaned off the sensors on top of the windshield really well. I'm suspecting something's actually broken at this point. Any ideas?

Cameras don't have that good a range, and often confuse lane markers with white/silver cars. Don't rely on the camera seeing things all the time. Your pic seems beyond where my 2020 easily finds cars (especially white ones).
Also, make sure you clean off the front of your radar (it's in the oversized emblem on a 2017+, don't know where on a 2016). The computer does its best with the radar pings to figure out what is a car vs a manhole, etc.
Also ACC will NOT identify a non-moving car, as the software has a really hard time differentiating between a painted turn arrow and a stopped car, so it intentionally ignores cars that are not moving to minimize false alerts. Even approaching radar pings of a stopped car are ignored, as the computer doesn't want false radar positives from things like rain-runoff canals crossing the road. It wasn't until 2022 that the software stopped deliberately excluding stopped cars.

If no front bumper accidents or error codes and the cruise control maintains its set speed, could ACC be turned off? You can turn the ACC off and back on by pressing and holding the ACC range button on steering wheel. Since I never done it, I don't know what is displayed in the M.I.D. with ACC off?
