Noise Cancellation

I can tell you that inadvertently getting in the car and closing the doors with my earbuds left in my ears from use at the gym feels like someone hitting both sides of my head simultaneously with bricks.
I've wondered if there are certain ways the audio needs to be used to enhance the noise cancellation. i.e. does it have to be on, is the volume level of audio important to noise cancelling, does the amount of sub in the mix change things, etc.???
I have the sensation that the car is most quiet when the hands-free phone feature is in use. Don't know if that is objectively true, but it seems that way to me.
