could be a loose connector on the instrument cluster, an improperly threaded screw to transfer the VSS signal from the cluster to the actual dial, any number of things. Does cruise control work? (note that it will not work below 25mph regardless of VSS)

if the D4 light is still blinking, the electrical troubleshooting maual has this to say: "When the powertrain control module (PCM)
detects an abnormality in the automatic
transmission control system, or when you request
diagnostic trouble codes through the service check
connector, the PCM will make the D4 indicator
light blink."
