'07 MDX Tech with 190K miles, targeting 200,000 km next year.
Bought it 9 years old with 115K miles from an Acura dealer in the Pacific Northwest. This is my wife's daily driving car.
The timing belt was done at 105K miles by a dealer in 2015.
Here is my list of repairs over 6 years of ownership and 75K miles (excluding regular oil, filters, fluid changes):
1. Bluetooth module failure. Dead battery and permanent disconnection. Lawsuits is going
2. Timing belt tensioner. It wasn't replaced by dealer timing belt job and became noisy on cold starts.
3. Brake discs/pads 3 times due to steering wheel vibration. Almost every 2 years.
4. All four wheels bearings / hubs assy.
5. Alternator. Died with a shorted bridge rectifier and almost smoky car harnesses, not to mention a dead battery.
6. AC repair - condenser, hoses, rear evaporator, o-rings, R-134A recharge.
7. Radiator replacement. As a preventive maintenance. The ATF cooler fittings were heavily rusted.
8. Water leaks into the rear sides of the trunk. Short circuits in the rear fuse box assy with all sorts of alarms and faults.
Repaired the fuse box assy and covered it with layers of plastic wrap.
9. Climate control servomotors replacement. Front and rear. Began to make noise from wear and in an attempt to find the programmed positions.
10. Power steering pump replaced 2 times and high pressure hose pending replacement. Noise and leaks.
11. Front and rear engine mounts replaced. Both were cracked and loose.
12. Door speakers replaced and stereo amplifier pending replacement. Crackling music distortion and popping sound when turned on.
13. Catalytic converters of both sides.
14. Side mirrors actuators replacement. Began to twitch / jam and the memory position of the seat / steering wheel / mirror did not work.
As current issues now:
1. Oil leak from oil pump gasket (oddly shaped o-ring between oil pump and engine block) - TB and oil pan needs to be removed. Waiting for next 200K miles TB service.
2. Slow oil leakage of rear main seal. Removal of transmission required. Awaiting replacement.
3. Oil is leaking from both valve covers. I'm waiting for the next TB service on valves adjustment.

281k on my 06. Just did the first two trans drain and fills, and changed rear diff fluid. Runs and drives fantastic.
I guess you changed the gear oil at the front of the driveshaft shown in this video? I missed this the first time I changed mine, it's hidden very well. I assume your 06 has this, my 10 does.
06 RBP Manual. Had it since 08 with 10kmi. LOTS of mods over the years, but to stay true with the OP's intent of listing items outside of normal maintenance (which i view things such as tie rods, motor mounts, and compliance bushings etc. as normal things over time for these cars):
*3rd suspension (stock to H&R/KONI yellow to Tein Flex Z)
*both drive shafts and seals
*both precats (replaced with HFCs)
*clutch
*cruise switch
*all four door actuators
*NEVER replaced ANY exterior light!
wow!!!!! That is amazing. I’m hopefully gonna be picking up a 2012 mdx with less than 75000 on it so it’s just a baby still but i only hope to get close to that kinda mileage!!!

This SUV is a BEAST! I’ve only had to do regular maintenance on mine! Got at about 146K strongly suggest to use Seafoam every 5k miles before. Changing out your spark plugs and your MDX will last forever!!
I have a 2012 MDX with the navigation package and I have reached 276,740! I think I should win a prize for having one of the highest miles. I’ve seen on this forum. lol
280,xxx 05 MDX touring navigation/entertainment pkg. No repairs that were necessary. Just my guilty pleasure to take things apart and put it back together. She still runs strong even with my aggressive race red light to red light mentality... It's just to fun to drive to be going the. Speed limit. It's even more fun to leave people disappointed when they try to keep up on these mountain twisty roads. Haven't felt the SH-AWD but the AWD system these 05 have is something I couldn't be more pleased with. Just before this I had the Lexus RX and I can't say night and day but far more impressed with the MDX handling... Heard complaints about body roll issues but this is one aged ride and I have never experienced any body roll coming out from dipping turns. Oh well love these cars. I'd marry it if I didn't like humans so much
