But if you can clearly see pad on both sides of rotor and no scoring on either side of rotor - something in the wheel bearings would be my next train of thought. Worst case scenario would be CV joints, but that would depend on the grind and when it was happening. CV's usually start grinding most when turning when they start to go out....which usually would be something you would have noticed before now.
Just some thoughts....

ck for the 3rd tab prob on the inner pads
supposed to be a raised tab on each end,,one holds the noise maker arm for low pad warning
the other is empty
IF there is a 3rd tab in the top center- its hitting the caliper piston -causing pad to run crooked
easy ck= get on ground to look at inner side of rotor
If it has half its width shiny and half rusty looking- not being used looking
you have the extra tab!!
several makers use the RL backing plate and sell it as a TL pad but its not
You can grind off the extra tab or swap out the correct pads
Will cause all sorts of noise- weak braking etc until fixed

did you clean and lube the slider parts where the pads move in and out?
flushed brake fluid in last 2 years?
flushed brake fluid in last 2 years?

Drifting
If just one pad worn, suspect caliper (sticking piston, slider pins dragging) esp. if you never replaced the brake fluid. Also chk. for improperly installed pads, lack of lube on sliding parts.
Who last worked on them? Brakeway, Midas, or other get it out door quick place?

if you did not turn the rotors or replace them then I would expect it to make a grinding noise as there usually is a lip on the rotor that is all rust and drags against the new pads when the brakes are applied most of the time it's not an issue but occasionally it is due to the fact not all pad sizes are the same some are smaller than others and if you installed one that is slightly larger than the old ones then you have this issue. look for the lip on the inside diameter and outside diameter relative to the pads id and od.
